Bisoprolol Fumarate

Composition :

Cardinor 2.5 Tablet: Each tablet contains Bisoprolol Fumarate USP 2.5 mg.

Cardinor 5 Tablet: Each tablet contains Bisoprolol Fumarate USP 5 mg.

Description :

Bisoprolol blocks the action of the sympathetic nervous system by blocking the beta1-adrenergic receptors and reduces the heart rate and force of contraction of the heart, thus lowers blood pressure.

Indications :

• Hypertension

• Angina

• Congestive heart failure

Dosage and administration :

Hypertension: The dose of Bisoprolol must be individualized to the needs of the patients. The usual starting dose is 5 mg once daily. In some patients, 2.5 mg may be an appropriate starting dose. If the antihypertensive effect of 5 mg is inadequate, the dose may be increased to 10 mg and if necessary, to 20 mg once daily.

Angina: Initially 5 mg once daily. This dose may be increased as needed and tolerated to prevent angina pectoris. If necessary, this dose may be increased approximately every three days to 10 mg and then 20 mg once daily.

Congestive heart failure: Initially, 1.25 mg once daily (in the morning) for 1 week. If well tolerated, the dose can be increased to 2.5 mg once daily for 1 week & then 3.75 mg once daily for 1 week, then 5 mg once daily for 4 weeks, then 7.5 mg once daily for 4 weeks, then 10 mg once daily; maximum 10 mg daily.

Side effects :

Fatigue, dizziness, headache, nausea, vomiting, diarrhoea, constipation or abdominal pain, cold or numb extremities e.g. hands and feet, muscle weakness, bradycardia, peripheral vasoconstriction, bronchospasm, dyspnea.

Use in special group :

Pregnancy: Bisoprolol should not be used during pregnancy unless clearly necessary.

Lactation: It is not known whether this drug is excreted in human milk. Therefore, breast-feeding is not recommended during administration of Bisoprolol.

Pediatric use: Safety and effectiveness in pediatric patients have not been established.

Contraindications :

Bisoprolol is contraindicated in patients with asthma, uncontrolled heart failure, prinzmetal's angina, marked bradycardia, hypotension, sick sinus syndrome, second or third degree AV block, cardiogenic shock, metabolic acidosis, severe peripheral arterial disease, sino atrial block etc.

Precautions :

Adjust the dose of Bisoprolol with caution in patients with renal or hepatic impairment.

Drug Interaction :

• Rifampicin accelerates metabolism of Bisoprolol

• Increased risk of bradycardia, AV block and myocardial depression when Bisoprolol is given with amiodarone

• Bisoprolol enhances hypoglycemic effect of insulin

Storage :

Keep away from light and moisture, store in a cool place. Keep out of the reach of children.

Cardinor 2.5 Tablet: Each box contains 3 x 10 tablets in Alu-Alu blister pack.

Cardinor 5 Tablet: Each box contains 3 x 10 tablets in Alu-Alu blister pack.
